Each person receiving services from M’agine Support Services, under the HCS program, has a case manager who coordinates the development and implantation of the Individual Service Plan and the Person-Directed Plan, integrates services delivered under the program, and records each person’s progress. People receiving services under the TxHmL program have a case manager within their local MRA.
